Patient care at our centers begins with a specialized evaluation to assess range of motion, pain levels, strength, and the ability to perform functional tasks. Our skilled therapists then establish a treatment plan focused on relieving symptoms and stabilizing the patient’s condition, with individualized goals designed to enhance recovery and return patients to function in the shortest time possible.

Upon arriving at the center for your appointment, you will need to fill out patient forms, which are available upon check-in by our front office staff. You can also download our patient forms, print them, and fill them out before your visit.

Please arrive at least 15 minutes before your scheduled appointment time and remember to be prepared with the following:


  • Dress Appropriately - You should wear comfortable clothes that are not restrictive to movement and closed toe shoes like sneakers (Ladies, no heels please) We can provide you with a patient gown to expose appropriate body parts, however, you may want to wear something that allows us to view that part without having to change (example: If you are coming in for a knee problem you may want to wear shorts or bring a pair with you to change into)


  • Prescription for Physical Therapy


  • Insurance card — if you are paying with insurance, please bring your current insurance card


  • Photo identification — you must show valid photo identification, which can be a driver’s license, passport, military ID, employment badge, or student identification card.


  • Current list of your medications
