Patient care at THERx begins with a specialized evaluation to assess range of motion, pain levels, strength, and your ability to perform functional tasks. Our skilled therapist will then establish a treatment plan focused on relieving symptoms and stabilizing your condition, with individualized goals designed to enhance recovery and return you to function in the shortest time possible.

Upon arrival for your appointment, you will need to fill out patient forms, which are available online or upon check-in by our front office staff. Please arrive at least 15 minutes before your scheduled appointment time and remember to be prepared with the following:


  • Dress Appropriately - You should wear comfortable clothes that are not restrictive to movement and closed toe shoes like sneakers (Ladies, no heels please) We can provide you with a patient gown to expose appropriate body parts, however, you may want to wear something that allows us to view that part without having to change (example: If you are coming in for a knee problem you may want to wear shorts or bring a pair with you to change into)


  • Prescription for Physical Therapy if required


  • Insurance cards


  • Photo identification — you must show valid photo identification, which can be a driver’s license, passport, military ID, employment badge, or student identification card.


  • Current list of your medications
